Applicants for the scholarships must meet and fulfill the following
requirements:

Reside in one of
the six counties CIEDC serves: DeWitt, Fulton, Mason, Menard,
Piatt and Logan. If you are a full-time student out of county,
you can still apply as long as your permanent address when not
in school is within the county.

Fill out a survey in the county where
you live. The survey includes family income for the previous
year and Social Security cards for everyone in the household.

Submit the
following:

High school
diploma, transcript of grades, GED certificate or SAT-ACT
scores.

Letter of reference
from last school attended.

Letter of
acceptance from Illinois school or training institute you plan
to attend.
-
Autobiographical
sketch, including a brief personal history, hobbies, interests,
career goals, educational objectives and how the scholarship
will be used.

One personal
reference.

no later than June 23, by 4 p.m.
