School and athletic physicals are also available from the Rural
Health Partnership at any regular locations and times except July
16-27 (schedules).
Appointments must be made by calling 732-9681.
The school and athletic physical fee for kindergarten through
grade 12 is $40, payable at the time of service to Family Medical
Center or with a current Medicaid card.

The immunization fee is $9 per immunization, payable at the time
of service to Logan County Health Department or with a current
Medicaid card. For kindergarten, fifth-grade and ninth-grade
physicals, students must bring immunization records. Completed
immunization records can be obtained from the Logan County Health
Department for $1.
