Fatal traffic accident on US Route 136

[December 01, 2008]  SPRINGFIELD -- Illinois State Police District 9 released the following accident report at 1:05 p.m. Monday:

InsuranceINCIDENT: Two-vehicle fatal crash

LOCATION: U.S. Route 136 one-fourth mile west of Logan County 1250th Avenue

DATE / TIME: Dec. 1, 6:45 a.m.

INDIVIDUALS INVOLVED: Driver (deceased), 21-year-old female, Bloomington, Ill.
Driver -- Timothy D. Miller, 39-year-old male, Joy, Ill.

ASSISTING AGENCIES: Logan County Paramedics, Lincoln Rural Fire Department and Rescue, Emden Fire Department and Rescue, Armington Fire Department and Rescue

NARRATIVE: A 21-year-old female was driving a red 1999 Mitsubishi westbound on U.S. Route 136 when she lost control of the vehicle. The Mitsubishi crossed the center line and entered the eastbound lane into the path of a white 2006 truck tractor driven by Timothy D. Miller. The Mitsubishi struck the truck tractor head-on in the eastbound lane. Both vehicles came to rest in the southbound ditch.

The 21-year-old female was transported to BroMenn Medical Center, where she was later pronounced dead. Miller refused treatment at the scene. The name of the driver of the Mitsubushi is being withheld pending notification of family.
