2-vehicle fatal crash on Route 136 near Logan County 1475 Ave.

[January 08, 2009]  SPRINGFIELD -- Illinois State Police District 9 released the following accident report:

Incident: Two-vehicle fatal crash

Location: U.S. Route 136 near Logan County 1475 Avenue

Date, time: Jan. 7, approximately 9:16 a.m.

Assisting agencies: Logan County Sheriff's Department, Rural Lincoln Fire and Rescue, Armington Fire and Rescue

Individuals involved:

  • Deceased male, 45, Armington; driver of white 1991 Chevrolet

  • Mary A. Crismon, 46-year-old female, Armington; passenger in white 1991 Chevrolet

  • Gary W. Myers, 41-year-old male, Bushnell; driver of gray 2007 Chevrolet

Narrative:

Gary W. Myers was driving a gray 2007 Chevrolet east on U.S. Route 136 when he lost control of his vehicle on the ice-covered bridge, approximately at Logan County 1475 Avenue. Myers crossed the center line and entered the westbound lane into the path of a white 1991 Chevrolet driven by a 45-year-old male. Both vehicles came to rest off the north side of the roadway.

The 45-year-old male was transported to OSF St. Joseph Medical Center in Bloomington, where he was pronounced dead. Crismon and Myers were transported to OSF Saint Francis Medical Center in Peoria with non-life-threatening injuries.
