To apply for these scholarships, applicants must meet and fulfill
the following requirements:

Reside in one of
the six counties the Community Action Partnership serves:
DeWitt, Fulton, Mason, Menard, Piatt and Logan. If you are a
full-time student outside the county, you can still apply as
long as your permanent address when not in school is within the
county.

Fill out a survey
in the county where you live. Information needed for the survey
includes family income for the previous year and Social Security
cards for everyone in the household.

Provide proof of
high school diploma, transcript of grades, GED certificate or
SAT/ACT scores.

Have a letter of
reference from last school attended.

Have a letter of
acceptance from Illinois school or training institute you plan
to attend.

Submit an
autobiographical sketch, which includes a brief personal
history, hobbies, interests, career goals, educational
objectives and how the scholarship will be used.

Submit one personal reference.
Applicants must have all information delivered to Community
Action's offices no later than May 17, by 3 p.m.
