HOPE Mobile offers school and athletic physicals

[July 12, 2011]  The HOPE Mobile is pleased to offer school and athletic physicals for the 2011-2012 school year. These physicals are available on days when the certified family nurse practitioner, Kathleen "Kat" Tucholke, is traveling on the HOPE Mobile, which are Tuesdays, Wednesdays and Thursdays. To schedule your appointment, call 605-5008.

The fee for the physicals is $45, available only for those making payments at the time of service, by cash or check. However, current Medicaid/AllKids cards will also be accepted and billed at the normal rates.

The following dates are reserved specifically for school and athletic physicals: Tuesday, Aug. 2; Wednesday, Aug. 3; Thursday, Aug. 4; Tuesday, Aug. 9; Wednesday, Aug. 10; and Thursday, Aug. 11. On these days, the HOPE Mobile will be parked at the Logan County Department of Public Health, 109 Third St., from 8:15 a.m. till 4 p.m. and will be closed over the noon hour. You must have an appointment scheduled for a physical on the above dates. You may schedule appointments by calling 605-5008.

Before and after the above dates, physicals can be obtained on the HOPE Mobile without an appointment on Tuesdays through Thursdays when the nurse practitioner is available. However, please note that due to limited staffing and resources, services must be provided only during open hours on a first-come, first-served basis.
