Abraham Lincoln Memorial Hospital Completes Community Health Need Assessment

[November 07, 2015]  LINCOLN - Abraham Lincoln Memorial Hospital has completed a community health need assessment and placed the document online for the public.

The nonprofit hospital identified four priorities for the people and communities it serves in Logan County and eastern Mason County. They are obesity, mental health, access to care and chronic disease management for cardiovascular health and diabetes.

Completing a community health need assessment is required of nonprofit hospitals by the Affordable Care Act of 2010. The hospital completed a previous need assessment in 2012.

To complete the need assessment, Abraham Lincoln Memorial Hospital collaborated with the Logan County Department of Public Health and other health and social service providers that represent the vulnerable members of the community.

“A multitude of issues determine the health of a community. No single hospital, agency or governmental body can be entirely responsible for the health of its community,” said Dolan Dalpoas, president and CEO of Abraham Lincoln Memorial Hospital. “We look forward to working with our community partners to address these critical health needs faced by many in our community.”

The community health need assessment includes an implementation strategy to address how the hospital will meet these priorities in 2016-18.
