 The rankings are built on data from a consumer
website (Medicare.gov) run by CMS. This organization sets and
enforces standards for all nursing homes enrolled in Medicare or
Medicaid. The rating is based on data gathered from regular health
inspections, nurse staffing levels, and quality measures. Analysis
is conducted on these three data sources and each of these ratings
is then combined into one overall rating.
St Clara’s Rehab & Senior Care received five stars, the highest
possible ranking. “The five-star rating recently achieved is
reflective of the excellent care and services delivered every day by
an enormously dedicated and gifted team of health care professionals
and support staff. We are so proud of this accomplishment,” said
Mike Eads, Administrator. Eads has overall responsibility for the
day-to-day operation of the facility, which is licensed for 106
beds.
St Clara’s Rehab & Senior Care is managed by
Heritage Operations Group. Heritage Operations Group, LLC is a
division of Heritage Enterprises, Inc., which was founded in 1962
and is involved in a variety of residential and health care services
for seniors. The company opened the first nursing home in
Bloomington, Illinois, where Heritage is headquartered.
