Driver -- Timothy D. Miller, 39-year-old male, Joy, Ill.

NARRATIVE: A 21-year-old female was driving a red 1999 Mitsubishi
westbound on U.S. Route 136 when she lost control of the vehicle.
The Mitsubishi crossed the center line and entered the eastbound
lane into the path of a white 2006 truck tractor driven by Timothy
D. Miller. The Mitsubishi struck the truck tractor head-on in the
eastbound lane. Both vehicles came to rest in the southbound ditch.
The 21-year-old female was transported to BroMenn Medical Center,
where she was later pronounced dead. Miller refused treatment at the
scene. The name of the driver of the Mitsubushi is being withheld
pending notification of family.
