[March 30, 2009]  The following recalls were issued by various federal and state agencies during the week ending March 28: 

InsuranceHIGH CHAIRS 

Fisher-Price is recalling 3-in-1 High Chairs sold exclusively at Target stores nationwide from December 2008 to March 2009. The seat can fall backwards from high chair frame if the booster seat release is unlatched while the child is in the product. Also, the seat back can detach if not fully snapped in place, posing a fall hazard and risk of serious injury. 

This recall involves the 3-in-1 High Chair to Booster™, which converts from a high chair to a toddler booster seat. It includes a removable tray, height adjustment and folds for storage. The product number (P5369) is printed on the side of the seat, on a label on the seat pad, and on the product's packaging.

PACIFIERS 

OKK Trading is recalling Baby Necessities pacifiers sold nationwide from August 2007 to January 2009. The pacifiers are banned under federal law. They failed federal safety tests when the nipples separated from the base. This poses a choking hazard. 

This recall involves Baby Necessities pacifiers with a blue heart-shaped mouth guard. The pacifiers were sold four to a pack. The words "Baby Necessities" is printed on the cardboard packaging.

TRAMPOLINES 

Aviva Sports Inc. is recalling Aviva Leaps and Bounds Folding Trampolines with Safety Pad ("L&B") and Aviva Pirate Jump ‘N Play Trampolines ("Pirate") sol nationwide from September 2006 to January 2009. The metal support legs of the trampoline can bend or break and the bungee cords that secure the mat to the frame of the trampoline can fray or break, posing a fall hazard. 

The recall includes two separate model trampolines. The Leaps & Bounds Trampoline is a 34 inch by 34 inch trampoline with a hold-on handle with model number 12731 printed on the pad that goes around the frame of the product. The Pirate Jump & Play Trampoline has an enclosure and model number 0840525.

CHILDREN'S FLEECE JACKETS 

Golden Grove Trading Inc. is recalling Children's Hooded Fleece Jackets sold at CVS stores in the Los Angeles area from August 2008 to September 2008. The jackets have a drawstring through the hood which can pose a strangulation hazard. 

The recalled children's hooded fleece jackets for ages 6-12 were sold in sizes M through XXL in black, navy, and heather gray royal blue. Brand names Golden Grove or Young USA, style number 20K, and RN number 106197 are printed on a tag on the jacket's collar.

CHILDREN'S SWEATSHIRTS 

La Jolla Sport USA is recalling O'Neill Children's Sweatshirts with Drawstrings sold nationwide from December 2007 to January 2009. The jackets have a drawstring through the hood which poses a strangulation hazard.  

The recalled children's hooded sweatshirts have RN number 85773 printed in the center back neck and were sold in sizes S-XL. The product name and style number are printed on the hangtag on the left side seam of the garment, which comes in a variety of colors.

CHILDREN'S HOODED SWEATSHIRTS 

Dysfunctional Clothing LLC is recalling Hooded Sweatshirts with Drawstrings sold nationwide from June 2008 to December 2008. The jackets have a drawstring through the hood which can pose a strangulation hazard. 

Lost Children's Sweatshirts with drawstring sizes S-XL, RN # 85773. The product name and style numbers are found on the hangtag at the left side seam of the garment.

HOODED SWEATSHIRTS 

MM Compound LLC (also known as Metal Mulisha Clothing) is recalling Hooded Fleece Sweatshirts sold at various retailers nationwide from August 2007 to November 2007. The sweatshirts have a drawstring through the hood which can pose a strangulation hazard. 

The boys hooded fleece sweatshirts have style number M47610315 and style name "ID" printed along with the RN# 120953 located on the country of origin and care label sewn in the center back neck and were sold in sizes S-XL. The sweatshirts were sold in two colors – black and white. A list of the retailers can be found at www.recalls.org

Rusty North America is recalling Hooded Fleece Sweatshirts sold nationwide from August 2008 to December 2008. The sweatshirts have a drawstring through the hood which can pose a strangulation hazard.  

The boys hooded fleece sweatshirts were sold with the following names and style numbers printed along with the RN# 123549 located on the country of origin and care label in the center back neck: Industrial R48210106, Continue R48210105, Warp R48210122, and Enuff R38203012. HOLIDAY LIGHTS 

Four Star Group USA Inc. is recalling Fresh Décor 10 Count LED Star String Lights sold nationwide at Safeway stores from October 2008 to December 2008. Electrical wiring in the lights can overheat, posing a fire hazard. 

The recalled holiday lights are a string of 10 LED bulbs, each covered with a clear, star shape. "Fresh Décor" is printed on the front of the product's packaging; the UPC (#7989390761) is printed on the back, below the bar code. 

Consumers should return the product to any Safeway store for a full refund.

CHILDREN'S FISHING POLES 

W.C. Bradley/Zebco Holdings Inc. is recalling Children's Fishing Poles sold nationwide from August 2007 to December 2008. The paint on the reels of these fishing poles contains excessive levels of lead. 

This recall involves Zebco Advanced Youth Fishing Rod & Reel combo sets. The reels are painted orange and have the words "Zebco" and "ZAVS 10" on the reel handle.

SPEAKER ADAPTORS 

Lenovo is recalling AC Adaptor used with ThinkVision Soundbars sold nationwide from October 2008 to January 2009. The plastic shell of the AC Adaptor can crack, posing a risk of electric shock. 

Lenovo sold the AC Adaptors as part of the ThinkVision Soundbar option. The FRU part number 89P8571 with date code 200810 is located on the back of the adaptor.

CORDLESS VACUUMS 

Electrolux Home Care Products Inc. is recalling Ergorapido, Pronto and Precision Vacuums sold nationwide from November 2007 to March 2009. The vacuum's battery can expand and burst, posing a risk of injury. 

This recall involves 2-in-1 cordless stick vacuums with a detachable hand vacuum and free-standing charger. Units involved in the recall have 9.6-volts printed on a serial plate on the underside of the handheld unit, and the following serial numbers: 074000001 through 085100001 and 74000001 through 85100001. FUNNEL PRE-FILTERS 

Outerwears Inc. is recalling Funnel Pre-Filters sold nationwide from June 2008 to February 2009. The Funnel Pre-Filters can create a flash when they are removed from the fuel tank posing a fire hazard. 

These filters are used to fuel-up off-road and racing vehicles. The recall involves Outerwears Funnel Pre-Filters, or fuel funnel pre-filters, with the model numbers 20-2465-03, 20-2466-03, 20-2467-03, and 20-2474-03. The red fabric mesh filters were available in two sizes, small and large, and in two different shapes to accommodate various sizes of fueling funnels. The name of the manufacturer, Outerwears, is the only marking on the product.

FIREPLACES 

Central Fireplace is recalling Direct Vent Gas Fireplaces sold nationwide from April 2001 to July 2007. Excess gas can accumulate in the firebox if the pilot light is not lit or has gone out. When consumers relight the pilot light, the accumulated gas can ignite with force and shatter the glass fireplace doors posing a serious laceration hazard. 

The recall involves Central Fireplace model CF3036MV-LP and CF3036MVB. The model number is located on the data plate behind the control panel door where the gas valve is located.
