            SPRINGFIELD – The Illinois 
			Department on Aging (IDoA) is seeking nominations for the 2015 
			Illinois Senior Hall of Fame awards. The annual award, established 
			in 1994 by the General Assembly, recognizes residents of the state, 
			ages 65 and older who excel in the categories of Community Service, 
			Education, Performance and/or Graphic Arts, and Labor Force. Since 
			its start, 95 people have been inducted into the Senior Hall of 
			Fame.

			 Nomination forms are available at local Area Agencies on Aging, on 
			the department website at

			or by calling the Department’s Senior HelpLine at 800-252-8966. 
			Nomination forms can be faxed to 217-785-4477 or mailed to:

			Nominations must be postmarked or faxed by Tuesday, August 25, 
			in order to be considered. 
			 
			Eligibility is based on the nominee’s past or current 
			accomplishments in the category of choice, which excel in one of 
			four categories: Community Service, Education, Performance and/or 
			Graphic Arts, and Labor Force. The candidate must be age 65 or older 
			and a current Illinois resident or a former citizen who lived in the 
			state most of his or her life. Posthumous nominees are also 
			considered.

  IDoA will compile a list of nominees then 
			consult with members of the Illinois Council on Aging to determine 
			the award winner from each category.  
			 
			This year’s winners will be inducted in the Senior Illinois Hall of 
			Fame during a ceremony in the Fall.
