 The screening is open to all families in the Mount Pulaski school
district with children who will be five years of age on or before
September 1st, 2016.
Please contact the grade school office at 217-792-7220 before April
15th to set an appointment time for your child.
Before entering in the fall of 2016, students need to have a current
physical and immunization record, dental exam, eye exam, and a
certified birth certificate on file in the office.
