 Anyone in the Mount Pulaski School District with a child who will
be five years old on or before September 1, 2017, should participate
in this screening. Please contact the grade school at 217-792-7220
to reserve a time slot for your child.
Students entering school in August must have a current physical and
immunization record, dental and eye exam and a certified birth
certificate on file at the grade school.
