[November 13, 2020]
Abraham Lincoln Memorial Hospital and the Logan County Department of
Public Health are asking Logan County residents to participate in a
2020 community health survey.
The survey is part of a community health needs assessment currently
being conducted in Logan County. The survey is available online and
in paper format through Nov. 30.
Every three years, Abraham Lincoln Memorial Hospital works in
partnership with the county health department to complete a
community health needs assessment. The process begins with a
thorough review of local health data, including items that impact
the length of life and the quality of life of residents in Logan
County.
The two organizations also review social determinants of health. The
World Health Organization describes social determinants of health as
the conditions in which people are born, grow, live, work and age,
as well as the complex, interrelated social structures and economic
systems that shape these conditions.

These could include discrimination, income, education level, family/social
support and community safety.
To thank the community for taking the survey, one individual will be randomly
selected to win a $250 gift card to Lincoln IGA provided by the ALMH Foundation.
The information collected will guide community conversations that will result in
a three-year community health improvement plan.
The survey can be found online at ALMH.org on the scrolling banner or directly
at https://tiny.cc/chna2020. For more information or to request a paper survey,
call 217-605-5008.
